If you live in our practice area and would like to register,

Please complete the registration forms available in reception or click the link below.

On registration, you may indicate to be registered under a particular practitioner. However, you will be registering with the practice rather than an individual GP.

Practice Appointment System

The practice operates an appointment only system.

Appointments may be booked during normal reception hours by our receptionists by telephoning 020 8795 7979/0208 795 7975 or Alternative if you have online access you may wish to book appointment online.

In Emergencies, patients will be seen on the same day. Please ring 8.30am for emergency appointments.

Please be considerate and let us know of any cancellations in advance so that other patients can take your place. Each appointment is for 10 minutes only. To avoid delays in your appointments please limit to one problem per consultations

Home Visits

PLEASE RING BEFORE 10:00AM WEEKDAYS

Home visits are available for those patients who cannot attend the surgery, mainly very elderly people and those are seriously unwell.

The Doctors will speak to all patients requesting HOME VISITS and will visit if the request is appropriate. If at all possible, patients will be requested to attend the GP Surgery to be seen.

Repeat Prescription

The practice requires time to process your repeat prescription requests. Please allow 48 hours (2 FULL WORKING DAYS) to prepare your repeat prescriptions.

Ordering your Repeat Prescriptions

There are a number of different ways to renew your repeat prescriptions. You can choose to :

  • Order Prescriptions Online
  • Drop the request slip in person
  • You can USE the NHS APP
  • Send the prescription via post

PLEASE UNDERSTAND THAT UNDER NO CIRCUMSTANCE WILL WE ALLOW REPEAT REQUESTS ORDERS OVER THE TELEPHONE!

If you are drop-in the request please using the repeat prescription form, please remember to tick which item you require.

If you do not have a repeat prescription form, please ask reception for the prescription request form.

Please note as of 31st March 2020 the practice will no longer be accepting faxed requests.

Rude and aggressive behaviour is unacceptable and any patients behaving discourteously will be removed from the list.

Protection & Use of Information

We ask for information about you so that you can receive the best possible care. We keep that information to ensure that any details presented to your doctor or nurse are accurate and up to date.

We only use or pass on information to people who have a genuine need to see it (referral to hospital/specialist, etc) with your consent.  You have the right to know what information we hold about you.  If you would like to see your records, please speak to our Administration Team.

Everyone working for the NHS has a legal duty to maintain the highest levels of confidentiality and the computer records we hold are protected under the General Data Protection Regulation 2018.

Medical Reports & Insurance Forms (by appointment only)

The doctors do conduct private insurance or medical examinations. This is conducted outside their NHS work and is based by appointment only.

Information Helpline (Out of Hours Services)

If you have an urgent medical problem please ring NHS 111 Service by dialing 111.
NHS 111 is free to call from both landlines and mobiles.
You may also attend the Brent GP Access based at Wembley Centre for Health & Care on weekends
(opens from 08:00 am to 20:00 pm). 

We also wish to take this opportunity to remind you that if you are very seriously unwell and require urgent attention, Call an Ambulance (999) or attend the nearest Casualty Department.

Carers Information

DO YOU LOOK AFTER SOMEONE WHO IS
ILL, FRAIL, DISABLED OR MENTALLY ILL?

If so, you are a carer and we would like to support you.  If you agree, we will pass your details to the Brent Carers Service, an organisation providing information and advice, local support services, newsletters and a telephone linkline for carers.

We will also refer you, with your permission, to have your needs assessed by Adult Care Services.
A carer’s assessment is a chance to talk about your needs as a carer and the possible ways help could be given. It can also look at the needs of the person you care for. This could be done separately, or together, depending on the situation. There is no charge for an assessment.
